第1, 2研究室(旧神経病理学研究室)は、脳血管障害の神経病理学を専門とされた亀山正邦教授により、1983年に京都大学神経内科に最初に設立されました。近年分子生物学、オミックス解析、遺伝子改変技術、さらにはiPS細胞の発見など、多彩な技術革新がなされ、疾患の病態理解に大きく貢献しています。しかし、神経疾患の真の解明にはヒトの脳を用いた確認作業が不可欠です。
当研究では、「ヒトの脳から学ぶ」をモットーに、神経変性疾患、脳血管障害、神経免疫疾患に焦点を当て、病理学、細胞・分子生物学、機能画像、検体解析など多彩なアプローチを通じてこれら神経疾患の病態解明と新規治療法の開発を目指して研究を進めています。
それぞれのエキスパートが進める研究進捗は、火曜日隔週に行われるカンファレンス(1,2研カンファ)でディスカッションされ、メンバー全員で疾患の多面的な理解を深めるよう務めています。また1,2研カンファは、研究進捗のみならず神経病理学や新規診断、治療法のトピックについてのセミナーも不定期に行っています。
研究室としての診療貢献は、入院患者の神経筋、脳生検の病理診断と剖検例の病理解析とCPCですが、大学院生(特に専門医試験前であるD1)には組織診断能力とCPCのプレゼンテーション能力を身につけていただくよう指導します。また関連施設の剖検依頼例については、特殊染色や免疫染色によって詳細に解析し、可能な限りCPCの形で結果をお返しします。
1. ヒト剖検脳を対象とした臨床神経病理学的研究
当研究室は日本神経病理学会の認定施設であり、豊富な剖検組織の蓄積があります。これらは、京都大学神経内科およびその関連施設で亡くなられ、剖検された方々からの貴重な献体によるものであり、私たちの研究室の歴代担当者によって神経病理診断され、パラフィンブロックは厳重に保管されています。これらの検体を用い、私たちは、筋萎縮性側索硬化症(ALS)やパーキンソン病およびその類縁疾患、血管性認知症とアルツハイマー病の発症機構の解明に取り組んでいます。総数は376例で、内訳は以下の通りです。
2. 神経疾患の神経病理学的解析(綾木孝、和歌山県立医大、滋賀医科大学との共同研究)
近年、神経変性疾患の多くは異常なタンパク質の蓄積が病態の鍵であることが判明し、神経病理学の果たす役割は
以前に増して注目されています。研究室の前々責任者である、和歌山県立医科大学神経内科 伊東秀文教授は広島大学や徳島大学、関西医科大学との共同研究で家族性ALSの新規遺伝子オプチニューリン(Optineurin; OPTN)を世界で最初に発見し、神経病理像を報告しました(Nature 2010)。また、研究室の前責任者である滋賀医科大学神経内科 漆谷真教授の元、家族性ALSの原因と一つである変異SOD1蛋白質がシナプス分泌蛋白質とともに細胞外に分泌されミクログリアの活性化や運動ニューロン死をきたすことを突き止め、ワクチン・抗体療法の有用性を報告しました(Nat Neurosci 2006, PNAS 2007)。
現在でも、和歌山県立医科大学神経内科、滋賀医科大学神経内科とは緊密な共同研究体制を構築しており、和歌山県立医科大学神経内科との共同研究では、中條西村症候群の筋病理像の報告(Neuropathol Appl Neurobiol.2020)を、滋賀医大との共同研究では、ALSの動物モデルに対する抗体治療の効果を報告しました(Sci Rep. 2018)。
当研究室は神経病理学、機能画像、オリゴデンドログリアなど研究室の研究指導者も充実してきており、多面的なアプローチも可能になっています。
3. Oligo-neuro-vascular unitに着目した神経疾患の病態解明、治療戦略(眞木崇州)
我々の研究グループでは、培養細胞、マウス脳、ヒト剖検脳など様々な研究対象を用いて、脳梗塞、血管性認知症、アルツハイマー病などの神経疾患の研究に取り組んでいます。
神経疾患を考える上で、神経細胞のみではなく、神経系・グリア系・血管系・免疫系を含めた神経グリア血管単位(Neurovascular unit、NVU)として、脳を包括的にとらえる重要性が提唱されています。
実際に、脳血管障害、認知症、神経変性疾患、神経免疫疾患を含む多くの神経疾患において、神経グリア血管単位の機能不全が、病態の発症や進行に密接に影響していることが多面的な観点から示されつつあります(CNS Neurol Disord Drug Targets 2013、Nat Med. 2016)。
神経グリア血管単位を構成する細胞の中で、我々はグリア細胞の1つであるオリゴデンドロサイトとその前駆細胞 (Oligodendrocyte precursor cell、OPC)に注目して研究を行っています。OPCはオリゴデンドロサイトの供給源であるということだけでなく、高い増殖能と遊走能を持ち、脳内外に存在する他の細胞種と様々な相互連携をしていることが報告されてきています (Clin Exp Neuroimmunol. 2017)。
たとえば、OPCの一部は血管周囲に存在し、血液脳関門の形成や障害後の機能制御において重要な役割を果たしうること(J Clin Invest. 2013、PLoS One 2014)、血管周囲領域においてOPCとペリサイトが近接して存在し、相互に作用することを報告してきました。(Neurosci Lett 2015、Stem Cells 2018)。また、脳梗塞後の皮質虚血領域において、血管周囲のOPCが増加し血管新生を促進することも見出しました (J Am Heart Assoc. 2019)。さらに、OPCの多様性の一つとして、OPCの遺伝子発現パターンや機能特性には性差があることを報告しました (Stem Cell Res. 2020)。
今後は、各神経疾患において、OPCがその他の細胞種とどのように相互作用し病態に関与するのかを解明することにより、新しい診断・治療アプローチの創出につなげていきたいと考えています。
4. 神経変性疾患の機能イメージングを用いた病態解析研究 (澤本伸克)
パーキンソン病やアルツハイマー病をはじめとする神経変性疾患を対象として、MRIや核医学などの画像技術を用いて病態解明につなげる研究を展開しています。具体的には、脳領域間の解剖学的、あるいは機能的結合を、生体内で可視化するMRIの解析手法や、アミロイドβやドパミン神経を可視化する核医学検査などを組み合わせることで、病理学的変化の空間的広がりや、これに基づく機能異常の側面から病態解明を目指しています。さらに、世界的にも貴重な7テスラMRIを用いた研究で、従来の画像解析では明らかにできなかった微細な構造異常を特定することを目指しています。このような最先端の解析手法を用いて、生体内での知見をもとに神経変性疾患の克服につながる、より詳細な病態解明を目指しています。
5. 神経免疫疾患のバイオマーカー探索、病態モデルを用いた新規診断・治療法開発研究(近藤誉之)
京都府立医科大学神経内科免疫グループと同一グループとして神経免疫疾患の臨床研究から基礎研究まで活動しています。 多発性硬化症(MS)を標的とする研究は臨床神経学のみならず、T細胞の抗原認識など免疫学にも大きな貢献をしています。多発性硬化症とその実験動物モデルの研究によって得られています。私達も免疫学の教科書に載るような成果を夢みています。 免疫系は免疫細胞と免疫関連分子が免疫ネットワークを形成し、お互いに制御しながら様々なバランスの中で生体防御をしています。MSはT細胞病、視神経脊髄炎(NMO)は抗体病とされていますが、免疫病態は実はより複雑です。神経免疫疾患の病態を免疫ネットワークの破綻として解析しています。 てんかん・運動異常生理学講座、神経生理学研究グループと自己免疫機序によるてんかんに関する共同研究をしています。病因抗体の同定できなかった場合の病態バイオマーカーを探索することが目的です。 同志社大学生命医科学部池川雅哉教授とプロテオミクスを用いた神経免疫疾患の共同研究しています。過去に私たちは視神経脊髄炎と多発性硬化症の蛋白プロファイルが相違すること(Annals of Neurology, 2012)を明らかにしています。
