我々「臨床神経生理学研究グループ」(第3.5研究室)は、システム神経科学の考え方と方法を用いて臨床病態の研究を進めています。臨床活動そのものの紹介はここでは割愛して、以下には特に研究面の活動に関して紹介いたします。
臨床の問題点に立脚して、神経生理学的方法によるアプローチに加えて機能画像の方法も駆使して、
1)運動・言語・感覚統合等の高次中枢制御機構の研究
2)ヒトの難治てんかんの診断と治療の研究
3)運動異常症の病態と治療の研究
からなるテーマを継続・発展させています。
これらの研究に関連しましては、専門医療機関(国立療養所機構静岡てんかん・神経医療センター)、他大学(東京女子医大脳外科、佐賀大学大学院工学系研究科システム制御)とも常時共同研究を継続しており、また学内では脳神経外科および高次脳機能総合研究センターと共同研究を行っています。
研究
1) 運動発現の中枢制御機構の研究
ヒトの固有補足運動野、前補足運動野などの運動関連野の機能:
難治てんかんの外科治療目的で脳表面に留置された電極から運動準備電位を検討し、ヒトの固有補足運動野は一次運動野と同様に運動準備電位の発生源であり、固有補足運動野内に準備電位の発生源としてsomatotopyを示すことを明らかにしてきました。
また前補足運動野も運動準備電位の発生源であり、身体部位のどこを動かしても共通した電極から準備電位が記録されることを見い出してきました。
さらに固有補足運動野と前補足運動野との差異を、背景速波活動の非同期化、随意運動の反復速度の違い、外的刺激に応じて運動を選択する機能(感覚運動連関)の検討により明らかにしてきました。
外側前頭葉では、発声・発語の準備機構、随意的運動抑制の準備機構、前頭前野における刺激の区別・判断・運動の選択に関わる機能を、それぞれフィールド電位の記録・解析から明らかにしてきました。さらに補足運動野症候群(両手の交代性運動が選択的にされる特異な状態です)に関連して、運動リズムと随意運動の研究を進めています(人見健文医員)。これらの一連の所見は、脳機能局在の検査方法として、現在脳外科の機能温存手術に臨床応用されています。
2)ヒトの難治てんかんの診断と治療の研究
a) ヒトのてんかん焦点の病態研究と臨床応用:
てんかん発作時に「発作時直流電位」と呼ばれる緩徐な脳波変化がてんかん焦点の持続性の突発性脱分極変位 (PDS)に相当し、神経細胞とグリアの機能を反映することは1960年代に実験てんかんで示されていましたが、臨床的意義は全く不明でした。そこで難治部分てんかんにおける脳表面あるいは頭皮上からの発作時の脳波活動を分析して、発作時直流電位がヒトのてんかん原性領域のうちてんかん原性がより高い範囲を明らかにすることを初めて示すことができました。てんかん手術の焦点検索でcore epileptogenicityを知る上で大変有用で、徐々に臨床応用されています。また現在てんかん焦点のおけるグリアの機能異常が注目されていますが、DC電位はグリア細胞の受動的脱分極とその後の細胞外カリウムの緩衝作用の障害が大きく関わっていることが示唆されており、グリアとてんかん原性、特に皮質異形成との関連から現在検討中です。
その他に、特殊な脳波検査、FDG?PET、MEG、MRI等のてんかん焦点検索における有用性を検討してきました。脳外科との共同での海綿静脈洞内脳波記録法はヒトの内側側頭葉てんかんの発作時焦点の検索に有用であることを明らかにしました。
脳磁場計測によるてんかん焦点検索では、脳内てんかん性活動の信号強度の大きさと脳磁場計測の感度の関係をヒトで明らかにして、臨床応用における所見の意義づけにおける基本的な知識をもたらしました。
b) 難治焦点の電気生理学的方法による焦点抑制方法とのその機序の解明:
てんかん発作の多くはチャンネル病であるという考えにも一部立脚して、内因性の皮質の抑制系大脳皮質の磁気刺激方法を用いて賦活することが注目されています。我々はさらに低頻度の皮質電気刺激、高頻度電気刺激、低頻度磁気刺激の方法を用いて、てんかん焦点のてんかん性活動の抑制効果と、脳波活動の変化を見いだし、それぞれの作用機序も検討中です(木下真幸子医員)。また頭皮上のDC電位を指標にバイオフィードバックの手法で、脳の興奮性を内因的に調節する方法がドイツから近年報告されましたが、その機序と臨床応用に関して、海外の施設と共同研究を進めながら精力的に現在検討中です。これらはいずれも将来的にヒトのてんかんの治療方法としての応用が期待されています。
c) てんかん外科および脳機能温存療法に関連する高次大脳機能の評価:
温存すべき大脳皮質機能を明らかにすることと密接に関連して、運動野の機能検索以外に、側頭葉底部言語野が日本人では漢字機能に重要な役割を果たすこと、ヒトの前頭前野・運動前野・一次運動野の機能連関の解明、全頭型脳磁図により海馬由来の聴覚事象関連電位を解析して海馬の機能障害の側方性を推定すること、second sensory area の生理学的検討、temporoparietal junctionの機能的解析、などを明らかにしてきました。
皮質間のネットワークの解明は機能温存上極めて重要です。そこで単発の皮質電気刺激により、ヒトにおける脳葉間の連結を特に言語野間、運動皮質野間で明らかにすることができました(松本理器医員)。これは新しい方法functional tractograhyとして、画像情報との相補的な解析をさらに進めている最中です。
d)特異なてんかん病態の解明
一次感覚運動野由来の「focal inhibitory seizure」や前頭葉由来の全身脱力発作の発生機序、補足運動野発作での強直発作の発生機序、全般てんかんの強直発作で脳幹が症状発現に関与すること、日本における家族性側頭葉てんかんの存在などを報告してきました。
3)運動異常症の病態と治療の研究
パーキンソン病での随意運動の発現の障害と、ジストニアでの皮質機能の病態に関して、パーキンソン病とジストニアでは随意運動における感覚運動連関機構が障害されること、ジストニアで随意的筋弛緩の準備過程が皮質レベルで障害されることを明らかにしました。
また特に補足運動野の障害にともなう運動異常症の病態は正常機能の障害か類推されるものの皮質ネットワークのもとで特徴的な随意運動障害を示す、これを磁気刺激法および脳電位分析から検討しています。
ミオクローヌスの皮質病態を脳波的に分析して体性感覚誘発電位の諸成分が独立して巨大化することを明らかにした。また振戦の病態が皮質性ミオクローヌスであった臨床例を皮質振戦(cortical tremor)として初めて発表し、後に本邦で多発することが判明した「良性成人型家族性ミオクローヌスてんかん」の臨床病態を明らかにしてきました。
また現在欧米では運動異常症の中で最も問題になっているpsychogenic movement disorderに対しても運動準備電位の点から鑑別方法を明らかにしてきました。さらにパーキンソン病のbradykinesiaの発現、安静時振戦の発現機構を検討中です。
4)その他
その他にも、ナルコレプシーの病態:Orexinとcataplexyの関連(clinico-biochemical correlation)、末梢神経障害におけるconduction blockの病態研究(人見健文医員)を、同時に進めてきました。てんかんに関する遺伝子多型の解明、てんかんのphenotypeとgenotypeの関連の解明(clinico-genotype correlation)は、神経内科内の分子遺伝学研究グループと共同で進めています。
